The Continuity of Care Document (CCD) is the result of the efforts of two standards organizations (HL7 and ASTM), to bridge the gap between Continuity of Care Record (CCR) and Clinical Document Architecture (CDA) adopters. How’s that for some acronyms?
The CCD can be thought of as a guide for sharing CCR patient summary information, as it uses templates for CDA elements to communicate data but the extent of data is determined by the CCR.
